Dying During Tryndamere Ult

Just played a game where I was killed multiple times during Ultimate as Tryndamere by Vayne. I suspect it had something to do with the bonus true damage from her silver bolts but I could be wrong.

During the Ultimate, Tryndamere is not supposed to be able to fall below 1 health by anything except Nexus Obelisk.
