I was able to learn Karma's third-level (final level) ult when I was at champion level 11.

You sure? Because she starts with 1 point in it at level 1 so it has 4 total point slots.

I had that once but it was with Khazix. I ended up getting all 4 Evolutions at 16

Karma's got 4 slots for that. She, Nidalee, and Jayce all start out with their ultimates at level one. Off of the top of my head I think that's all of them.
