Kled W not proccing Hydras/tiamat

I've been playing kled a lot for a while now and I've noticed that when Violent tendencies' last hit is used it sometimes refuses to proc any of the hydra aoe effects.(tiamat, ravenous hydra, titanic hydra) Titanic hydra active also sometimes breaks and does not apply to the fourth attack but instead applies on a regular AA.

I am completely unaware on how to reproduce this effect as it has been totally random but way too Common. It has made me lose plenty of flanking opportunities and it's making real mad, please look into it.
