Vi's Q unusable?

Hello guys, hopefully you all are enjoying the new rift look as much as I am. With a nice update bugs are sure to come across every now and then... and sure enough this is one of those times.

For three games now in Team Builder I have been a Jungle Vi with the matchmaking modifier on for newer players to the champ. In these three times, I've had trouble with her mid to late game. As soon as I die for let's say the 4th or 7th time (yes, bad k/d QQ) upon respawning I find myself unable to use her Vault Breaker attack (Q). It is not on cooldown, when I highlight my mouse over it, the arrow showing the path appears, but when I click, nothing. When I press Q, nothing. So for the remainder of the game I have to rely on my other three skills (one of which is passive) and good old summoner 4 to get in and out of battle.

Has anyone else experienced this issue?

With the smart cast system you have to hold down "q.." But I have yet to see that with her and she remains my favorite and top jungle.

Champions that charge in like Wukong with his E, Zac with his E, etc., in my experience, lose that "movement" ability when they get hit with CC as they die. I have no idea how to fix it. It's pretty awkward explaining to your team, as the tank, that you can't engage as Zac.
