Malz QSS and Fiora or Malz and QSS in general

Was just playing in a game where every time I would ult Fiora as Malz she would use QSS and come out of my ult, which is normal. However my ultimate would no longer do the damage to her. I would stay ulting, but my ult would no longer damage her. I am wondering if this is a QSS issue or a Fiora issue.

Please look into this as it is very frustrating when you are clearly beating her, but that bug causes you to fall off HARD late game.
