Bug In Client

I was qued for a ranked game and the game popped up, i quickly hit accept then tabbed back to my youtube and the game stopped responding, this forced me to close the game but when i reloaded the game it put me back into the champion select with all blank slots and i couldn't click anything, and when the timer next to my "slot" was up it made me dodge gave me 5 minute leaver and took 3Lp.
