My Screen is Going Black at Champ Select

Two times in a row just now, I was queued up with a full party of friends and when we finally got into champ select, my screen went black and I got 2 dodge timer penalties. I sincerely hope a solution is found as I noticed that there's a similar thread from a few years ago that has since been closed and unable to be responded to so I decided to make a new one considering this is still an issue. I've included a screenshot with this. If someone could get back with me about this, that would be great. I'd like to see something done about this sometime soon. Thank you.
