I was fighting Syndra as Anivia with help from Vi. I died from Syndra ult with my egg passive still up (my passive did not proc during the fight). I am almost positive that I was also using my Q when I died.
The screen shot was taken directly after i spawned and it shows that Anivia's passive is still up.
Later in the game i was killed by Lee Sin and and Anivia's passive did proc.
My passive did NOT go on cd.

Edit: I also remember you specifically saying "let's egg-bait her" and I checked and saw your "egg timer" buff wasn't on you.
As an anivia main I can attest to this bug. I have no idea how to recreate it, but I've died with my egg passive still intact multiple times. It happens rarely, but it's happened enough for me to know I'm not crazy!
I actually had this same bug happen to me sometime during this past week, but forgot to report it as it happened early in the match. I was doing an ARAM match, and was pretty sure I had egg, the enemy team tower dove and killed me, but my egg either never materialized or they did so much damage that they killed it instantly and I never saw it. My passive went on CD though.
I can attach logs if you want, I'll just have to go look up when that Anivia game was first (I don't believe I've played her since). Also, if it helps, I may have been Singed flung and killed while midair.
as an anivia main, i can also say this has happened to me as well. interestingly, it happened vs. a syndra as well. very rare though. out of 800 games, probably has happened only 5-10 times.
maybe you died while your passive just came up within milliseconds? and you're like : i had passive wtf? just a suggestion. its happened to me before. i should be more aware of my egg timer xD being an anivia main and all.
Maybe it's a visual bug with the icon, so that it looked like it was off cooldown, but was not actually. I recently had that happen to me with Janna's ult, it showed that her ult was off cooldown but when I pressed "r", nothing happened. I kept spam clicking it and eventually it worked... while I was in my own base...
But anyways, did you die previously? If you hadn't then it isn't what I described above.
