[Survey] Survey pop-up had no contents and auto submitted

Hello,

After a match I received a survey prompt (presumably over the new champion), however this prompt had no contents and when I attempted to click on it it then sent me to a webpage stating that my feedback and been submitted despite nothing having been selected.
