[GAMEPLAY] Katarina bug: Q, B and R stop working.

This has happened to me a few times while playing Katarina but I'm not sure how exactly it happens. I searched around a bit to see if others have the same issue and people report that after engaging an enemy and nearly dying you lose the ability to recall, cast Q and cast R. The only way to solve this would be to die, rebooting the client or resetting keybinds did not help me at all. I'm not sure if it happens all of the time, but when it does happen it renders you useless in a teamfight. I'm not entirely sure of the steps but so far it seems like to be like this:

1: Engage Enemy 2: Come close to dying while engaging enemy with or without full skill combo

There's a video I linked of someone having a similar issue, though it's after he/she engaged Cassiopea. It's not limited to enemy champions because this recently happend to me with Ekko.
