Getting Mastery Level 5 and a Mastery Level 6 Token in the same game

A friend of mine and I just got out of a game where he got mastery level 5 on Miss Fortune, and got an S in the same game. However, he didn't get a Mastery Level 6 token. Is this because it rewards you in a set sequential order, where the token comes first?

Same happened to me. I just finished playing a Zed game and got level 5 from it plus an S-. It showed before I left to the main screen that I got the token but it didn't show up in my crafting inventory.
