Game doesn't connect to lobby

Queued up for a game, hit accept and switched tabs out to look at something, when I tabbed back I was met with this screen and was hearing the various sounds of champions being banned. I then got kicked back to lobby with a dodge timer and presumably some LP lost. I also received a friend request at the exact same time as queue popped, not sure if that's relevant to the glitch.
