Fiora with 2 vitals one a bug the other real

During lane phase against a gang plank it started off well got 1st blood. then 2 vitals were showing up, one would be the real one the other just stayed there. it made lane really hard and basically gave GP a free win. I was using the project Fiora skin. Was not sure if anyone else is getting this or if it was a one game thing. Have not been able to recreate it. Also that game everyone was getting some ping spikes on our team.
