Not Earning Hextech Chest....
After my entire team earned S I still did not receive a hextech chest... can someone explain as to why?

Do you have chests available? You can only store up to 4 at a time and they reset pretty slowly (you get back only one a week). You can view this under Profile -> Overview -> hover over the chest icon under your name and it tells you everything you need to know about it.
On the other hand, in order to earn chests you have to own the champion you play (so it can't be one from the Free Rotation), and you have to get at least an S-. Teammates don't count, they can have any rating after the game, as long as you get rated S- or higher and have not earned a chest on your champion previously, you should be eligible to get one.

In addition to what Varys already wrote: You can also get a chest if your premade got an S- or better. So it is possible to have a chest on a champ where your best was anything but not an S
