Accept/Decline screen not appearing forcing a dodge

After having random freeze-lag in-game I attempted to queue for the next game and was never notified of the pop-up to accept. Thought I must have clicked off it at the perfect moment and just queued again. This happened 3 more times causing me to not only lose my series but have to wait the 25 minute wait timer for dodging too many games. The accept/decline screen simply didn't appear. The last time I saw the count stop at 1:01 with no pop-up. Thinking it may be a visual bug i tried to click where it would appear and nothing happened until it told me I declined yet another queue. Was not recording considering I was simply dumbfounded by the occurrence.
