Connectivity Issues after Freezing in Game

When I get kicked out after League freezes on me, I attempt to sign is as normal and it says the log in is successful but then after some time loading/thinking an error message will come up. This has been very common since at least the most recent patch.

I have frozen quite often lately during League, especially during ARAM matches or the holiday Poro map. Immediately I can think of 4 times in the last week in which my computer freezes up on me and only one of those times I was able to Alt-Tab out of the game (all other times I could not even Ctrl+Alt+Del and was forced to hard reboot my computer).

I wanted to post the screen shots I've taken and while I am unable to produce any of the times when my computer freezes in game I do have some to share.

I attached two screen shots... one in which I successfully log in and should join the server and then the follow up message I get shortly after. Clicking "here" has not been very informational (so much so that I forgot to write it down and include it here xD) so I thought I'd post it here and see if anyone else knows what's up.

PS: Even if this is not successful in providing me with an answer this is my sort of "I'm sorry" to all my fellow team mates as it takes me 8-10 minutes to get back into game after hard rebooting, then juggling with the "yes I'm in, wait no I'm not" issues that my computer seems to be having with the LoL client with this patch. Only one time have I managed to come back and have the game still in progress. I'm happy to report I ended up winning that match :-)
