Warmog's Passive
Warmogs passive activates at 3001 health rather than 3000 like the tooltip suggests. I only had exactly 3000 health once on ARAM and I did not get the passive until I had more than that.

It showed 3k hp but what the game will do it round up. Which means your hp could have been between 2999.5-2999.9 which means no 3k hp so no super heals.
