Smite Charges not stacking

I don't have any video of this occurring as I reinstalled my OS last night and I haven't installed plays.tv yet. That being said I have been playing all day and this issue has only occurred in one game. About half way through the game I noticed that I rarely had smite up and started paying attention to it and realized it was not holding 2 charges. It would come off CD and would not start the CD for the second charge. I have no idea how to recreate it or what caused it. Just thought I should let you know of this issue.
