Flash unusable after CD of hexflash

I do not know how this happened, I was playing leona and using hexflash. About halfway through the game i noticed that my hexflash was not up and my flash was grayed out. When I hovered over and pinged my flash it said it was ready. I could not use the keybind or click it. Here is the exact sequence of events as my hexflash and flash were coming off CD. I am playing leona, items I have are

1. Mobility Boots
2. glacial shroud
3. Remnant of the aspect Trinket = Sweeper
4. control ward
5. Crystalline bracer

Hex flash CD has 10 second remaining when the normal Flash cooldown kicks in with 2 seconds remaining. So CD goes from 10 for hex to 2 for flash instantly

The following happens to my character (leona) I am affected by Brand E Rammus Q Morgana E I then use Leona Q

Flash is now off of CD but it is grayed out. I could not use it for the rest of the game, even after relogging.

Please let me know if there is any more information or if you would like a video recording of this happening, this is a pretty game breaking bug.

This happened in a solo/duo ranked on summoners rift.
