Champion Selection Screen display error, then crash.

So I got into a game with my party and after selecting my champion before ban phase the GUI goes weird { Champ Icons go away, as well and summoner spells Champ Picture in center is missing My player menu is missing Chat is moved about 3/4 off the screen }

then after i banned a champ the game crashed. also i have a screenshot but don't know how to put it with this. this has happened twice to me now, is there something wrong i need to fix on my end?
