Yorick's Omen of Famine misses target [GAMEPLAY]

I have encountered this glitch several times over the past month and it has had an impact on several things, but mostly laning phase. What happens is Yorick's E - Omen of Famine, will sometimes spawn a significant distance from its target. It will still damage the proper target and restore Yorick's health accordingly, but the ghoul itself will spawn at a different location where it usually can't do any damage. I haven't determined exactly what circumstances create this glitch, so the only steps I could think of would be picking Yorick and spamming E until it happens. In the attached video I used Q and W to catch up to the target Vi, but I'm not certain if the glitch occurs only under such circumstances.
