Rek'sai Bug

I couldn't snap a screenshot at the time, but there is a bug with rek'sai's tunnels. One of the tunnels could not be destroyed my team while the enemy rek'sai could still ult and teleport to it. It had only one side to it (no output). No idea would could have caused the problem (maybe she died while in the tunnel).
