Sometimes Cho'Gath's Rupture is completely invisible. In the linked game, I (Annie) was killed with the help of a Rupture that never showed in my game. I didn't realize it was there until it had knocked me up and killed me. Cho'Gath was not in the fog of war either.
Yeah, used to happen even more often. I mained cho for two seasons and heard this complaint often. Not sure how it happens, but it's really annoying on the receiving end.
I have had this happen with Illoui, both grabbing or getting grabbed by her spirit ability. So long as she is in the bush you cannot see that she has your soul until she damages it and the rage indicator isn't there for how far you can be.
