GAMEPLAY Ashe Arrow Disappearing

When bot lane I have had my ADC's Ashe's Arrow just disappear as soon as she shoots it. It has happened in two different games now. You see the arrow start and leave her, travel for about .2 seconds then just disappear. No one is ever hit by it and it doesn't appear on the map or anywhere else. Her cool down is also like that of is she just fired it.
