I got this out of a chest and i noticed it's name wasn't showing correctly, figured i'd put it here.

The only way you can recreate this is to get it out of a chest i guess.

Ya, that's a known bug they are working on. You can craft it but it won't show in your ward inventory for now (but you'll still own it), and there are several wards with this issue atm. Not ETA on a fix yet, but when it rolls out any of those invisible wards will show up in champ select again.
