I keep getting queue banned

Ive gotten queue dodge penalties 3 times today, once was 10 minutes for not banning a champ which I did in fact miss, but it said due to recent (i havent played much at all) I got a 10 minute right off the bat! but still , no huge deal. Then I was in champ select and all of a sudden before the game started, I brought me back to the main page and said i failed to select champ (WHICH I DID SELECT AND LOCK IN !!) AND i had a 30 minute ban. got off for a while, played again later it was fine then a few hours late (just now) same thing happened and another 30 minute ban. Im hoping it doesnt start kicking me off longer than this since I am not actually dodging and not selecting, I am starting to get annoyed though,I was playing poros so 30 minutes is a game or 2 . Im not sure why this is happening to my game and its not registering me!
