Akali Shroud Visual Bug
I don't know what caused this, but it was 100% consistent the entire game. If I pressed W, then it looked just like in the clip below. It made the champion almost unplayable given her entire kit revolves around weaving autos with your shroud to proc passive. Probably the worst experience I have playing league since I began years ago.
