Quinn bird form bug

My friend decided to play Quinn and like halfway through the game he went into bird form. He was then unable to ever get out of it for the rest of the game. He was only able to use bird skills except his ult, which doesn't show the animation but produces the sound and no damage to the opponents around him. The video that i linked contains about 3 minutes of game play just a little before the transformation and him unable to come back to human form. If you require the entire video let me know but 3 minutes is way more than enough to show that the character wouldn't swap back in human form.
