Kicked out of bans for no reason
Have gotten kicked from games when picking a ban a couple times recently, will click on a ban then get kicked from game and message comes up that I dodged, and I have to wait 5 mins. Is it a bug?

yea i have, but it was while i was selecting a champ and i had like 5-7 seconds left
If you didn't Lock In your picks/bans, then no, it's not a bug.
I've been having the issue of not being able to lock in my pick no matter how many times I click the 'Lock in" button. Then I have to wait 5 minutes because Champ select has too many bugs in it. Fun times.
