I can't see the timer, but I'm assuming it reads 0:04 - 0:05 before it crashes. Are you currently running Windows 10? If so, read this. If not, you'll need to post more information for Riot to:

More information includes your computer's specs, champions involved, and relevant logs from the logs folder (assuming Windows: C:\Riot Games\League of Legends\Logs\Game - R3d Logs).
