FPS drop only when FPS is capped?

Uncapped on max settings I always get around 200+ fps, but when I cap it on 144, it sometimes drop to like 100 but it literally feels much worse, it feels very choppy almost like my character is skipping. Is there a fix for this? I know I could just play uncapped but something has happened during one of these patches where the fps is messed up with vertical sync or capped fps.
