Akali Passive Circle Bug

Of recent Akali's circle passive isn't proc'd on ability hits, specifically, Q and R2, Additionally, when I sometimes get a circle passive with Q, it centers on me or centers exactly between myself and the Q'd enemy instead of centering on the enemy. Is anyone else experiencing this?
