Dynamic Queue - Pre-picks not showing

I have begun to notice that very rarely in dynamic queue the pre-picks from my team would not appear for me and that it still looked like they had not chosen a champ. This causes me to sometimes ban my teams champs and obviously leads to some pretty upset teammates. Again this is very rare and barely happens to me so I was wondering if anyone else has experienced this as well.
