Old ability icon in death recap.
When I got killed by an Evelynn, I oepned up death recap to see the dmg and sure enough she used E on me, except the death recap showed her old E, as in, before her rework.

It must be remembering the good old times, when it didn't have to put up with so much shit that it can't tell apart.
