Lee Sin's Spellbar R bug

One day I was playing lee sin and I saw a pesky fiddle wandering around in our jungle. I decided to teach him a lesson, so I went looking for him. I found him and i tried to teach him a lesson but i died. The thing is, when i tried to kick him with my ultimate, my animation went off but it didn't do damage, basically I died before my ult could do damage. After I died the spellbar showed me that my ult is not on cool down. Normally if your spells are on cool down, the spell bar would read the time limit but mine didn't. So after spawning and thinking that i still have my ult, i went on to find Nasus on top lane to pick a fight. He was slaughtering me, and wanted to use my ult to get him away but the ult didn't go off even though the spell bar read that i still have my ultimate(yeah I died again -_-). This was the second time I experienced this. So anyone else experienced this?
