I got an S+ and didnt get a token did i do something wrong?

What game mode were you playing? Tokens can only be earned on normal and ranked Summoner's Rift, so if you were playing ARAM, Twisted Treeline, or a rotating game mode, then you were unable to earn a token for that game.
