Katarina~ could not use Q (Bouncing Blades), R (Death Lotus) and could not B (teleport back to Base)

After going 3/1/~ for Katarina, I immediately bugged out after getting my 3rd kill and I could not use my Q or R anymore and I could not even B to teleport back to base.

The process of which it happen:

1.I started with and 4 2. 3. 4.Got my 3rd kill and bugged out 5. 6. Restarted client a couple of times. 7. Still bugged so I sold 8. Died and still bugged. 9. Late game, I bought , still bugged. 10. Died again (3rd death) finally not bugged anymore

I was on the other team in this game, it really turned the game around in our favor and I kinda feel bad for taking advantage of it (but it's ranked, I'd do it again _) He submitted a picture, but here's a direct link to the match history http://matchhistory.na.leagueoflegends.com/en/#match-details/NA1/1759171655?tab=overview Since kat didn't get any changes on the surface posted in the patch notes, **I have a strong feeling this may be related to the new 120ap item ** (or some hidden game engine voodoo magic no one told us about)

*Just in case, to dispel anyone trying to claim the player may have been raging at the team, the player was asking to get reported in hopes it show up in the tribunal for someone to see, as they didn't know about the bug report section. It's very sad to see a kat frantically running around in a team fight unable to do anything, especially when the kat's team is ahead by a fair amount (game history said 3k). *

It's not just Kat. I have had this happen on multiple champs.
