[CRASH]Screen truns to black after game start loading.

Right after the Champion selection when the game start loading, my screen turns to black and my driver stop working. I highly suspect it is due to my forth monitor's irregular resolution, but I wish there is fix to it.

Here is the hardware I am been using: Hardware: 3 1080P 3D surround monitor + 1 2560X1080 ultra wide monitor 3 X GTX 680 GPU 1 X i7 980X CPU

OS: Windows 7 Ultimate X64 All my driver and software are up to date.

The game works after I disabled my forth monitor. But I wish the game will work without this painful step.
