Vel'koz Ultimate unable to be used after lvl 11

Playing ranked game with vel'koz. Everything else in the game was fine before but after level 11 and second rank of Ultimate i was unable to use it the eintire rest of the game. Dying did not reset, nor did putting the last rank into it.
