I am being placed in penalty queue every time I try to get in queue

I am being put in penalty queue every time I try to get in queue. And this happens for no reason... always have to wait 5 min. But when I try to play after those 5 minutes, I am being placed in penalty queue again... Haven't been playing for the 3 past days because of this issue.

Not sure if you dodged/afked in a game or something? Normally, the penalty queue lasts for several games though. I have no idea what the minimum penalty is though.
