Anivia Passive Bug

So, I was playing Anivia (had played Anivia 2 rounds the day before as well), Blackfrost Anivia to be specific, when Warwick ulted me, and it was like the passive was skipped. When I respawned, I was egg form, could move around, cast abilities, and when I was standing still the animation for rebirth would just repeat. Couldn't basic attack, and I tried to relog, I tried to execute from turret in-game, but nothing helped so we surrendered. I really think it's annoying that we had to lose because of a bug, so I thought I'd report it here, and see if you can get i fixed. I also recorded this round for a YouTube video, so you can watch it here (it starts about 10 min into the video). I know that the game is currently full of bugs because of the new map, but I hope you can fix this. Thank you.
