When I was in a ranked queue today a player did not lock in their champ. I saw others leave the room and could even type in chat, but I could not. After a few moments I received the PVP.net disconnect message and it kicked me out of queue and gave me a dodge penalty.
My internet was fine and it was the player below me that did not lock in, so I believe this was a bug.

Same issue here. Happened to me multiple times already. You can repair your client from the launcher which fixes it temporarily, but the same problem came back after a few days for me, so be warned.
Happened to me too, got a 15 minute leavebuster. I don't even think they are dodging, I think it's just a bug in the loading screen menu. I thought it was me, so I uninstalled league and reinstalled. Then I still had the same problem. So I create a new partition on my hard drive and am still having the problem. It's gotta be a bug.
For me, on new champ select, it's impossible to select a champ, because the selection timer stops at 0 then freezes, followed by "an unknown error has occured"
