No one needs to physically disconnect from the game in order for a remake to happen, someone only needs to be marked as AFK for at least 90 seconds. Considering that the Warwick is level one with no CS, damage dealt, or damage taken, he likely just stayed in base the entire time which is enough to trigger a remake vote. Maybe he was having connection issues shortly after purchasing his items, and probably managed to finally reconnect around the time the remake was going on. Doing this will allow him to vote on the remake even if he was the "leaver" who triggered it. Just know, though, that the Warwick did indeed receive the "leaver" penalty of the remake. He lost LP and got that game flagged by Leaverbuster.
Also, first blood will only prevent a remake from happening if it happens before someone disconnects or is otherwise marked as AFK. Once someone is marked as AFK, though, first blood will do nothing to prevent a remake from occuring. First blood in this match happened at around 3:26, which means it happened while the remake vote was going on, so there is no way it would prevent the remake.
Lastly, a remake vote only needs 30% of the votes to be yes in order to pass. 2/5 votes is 40%, so that is enough for it to pass.
None of this is a bug in the remake system.
