Sion's Q and R Attacks Stop Working

During the game, Sion's smash attack (his Q) and his ult (his R) will stop working and will not work the rest of the game. I'm not quite sure what may be causing this as it seems to happen at random and it only seems to be a new problem. It has happened in two games I've played recently and it seems that others are having the same issue.
