Volibear passive

This happen to me yesterday as well as today voli's passive doesnt activate sometimes early in the game i was killed at my red buff 2 min into game it didnt activate even though i ran away at less then 100 hp as well 1 min 12 sec later when they invaded me again at grump it didnt activate till the third time i was taken low. The game was at 9:30 pm PST
