Occasional game start up, with hp bar blacked out, and a 5th champ portrait above mini map

This happened as the game started and dying would not get rid of it, as well as the hp bar being completely black, but was still numbered correctly. however i also had around ten hp more then i was supposed to have, 645/635. it didn't change until after i turned level 8 when the extra player portrait disappeared, and the hp bar stayed black the whole game.
