Quinn: Stuck in valor form and can't use ult, even death didn't fix

So recently I just played a game of Quinn ADC, and in the middle of the game, I was killed and revived in valor form. So at first I thought that I had accidentally used ult but then there wasn't any timer to see how long I stayed in this form. Five minutes later, I was still in valor form and couldn't cast my ult again, when I was killed, I would respawn as Valor yet again. The whole game, I was Valor so I bought ravenous hydra which worked very well. All my abilities were in ult form but my passive had the animation where the bolts were coming out as if I was in range form.
