There is almost a 50/50 chance of it not proccing the healing part of her ultimate. Generally, if I ult someone and it's been on them for roughly 3+ seconds, it's guaranteed to work, but any shorter and it won't. I've put it on someone within half a second before they even die, I've put it on them and auto attacked one of the vitals, then Q'd the other which killed them, but it happened so fast, my ult still doesn't proc the heal. It's extremely irritating...
Did you proc at least one vital on the ultimate? When the target dies with Fiora ult on, it requires Fiora to have proc at least one vital to leave behind a healing zone. I totally though it was broken until I read the tooltip
This happens with Twisted Fates Blue/Red/Yellow cards as well and flash is bugged right now that a second or two after you flash the game thinks your player is still where it used to be and you can be auto attacked.
Bugfix Fiora so she can be nerfed pliez :3
Not only have I had this happen (I feel like the vitals must not be strikeable right away). But it also feels like, if you lunge too close to the center of their character model, you end up INSIDE the vitals and don't hit any. That could be an illusion, but I swear it feels like it happens)

This happened to me several times. It's exceedingly annoying, especially when you're expecting an enemy gank and the heal would've saved your life.
