Steraks Gage Bug

I was playing ranked soloq graves jungle. I build steraks gage and the item didnt work once the entire game. After i completed steraks we went to siege enemy inhib tower and i got bursted, i was confused why my steraks didnt proc, my teammates said maybe their burst was just too big and i couldnt see the shield. It was possible but the weird thing was that the item didnt go on cd either. btw i also had maw and that did go on cd. 2nd the item shouldve proc'ed, I lived the fight with 10% hp but the item still didnt proc that fight. 3rd and last time when we pushed for the finish i went solo on enemy adc, I was full hp so was he and I died w/o having my steraks proc else i would have killed him. So the item Steraks Gage wasnt working properly that game. I dont know if this bug is in all the games but it was in mine. So riot plz look into it.
