Lucian Passive doesn't fire second shot---- PROJECT Lucian

I have played PROJECT Lucian multiple times the past few days and his passive sometimes will not fire the second shot. I can pull up past streams if need be. I am not sure whether or not its just the skin as I have not played the classic skin but AT LEAST on Project Lucian skin this is happening about 5-10 times per game.

I have had this issue as well while using the same skin. I also haven't tried using his default skin. Never noticed it in a fight but while farming it tends to happen often.
